What age groups are served?

Ages 6 – 65.

What insurance plans are accepted?

We are currently credentialed to accept:

Blue Cross/Blue Shield, Aetna, Tricare, Optum, United Healthcare, Cigna, Cisco, Humana, Golden Rule, Oscar, Anthem, UMR,  ASAGEHA, AllSavers, and more.

How much will I pay without insurance?

New Visit $350, Established Visit $150. Payments are due at the time of service, via the Therapy Portal. How do I start my visit? You will receive an email invitation to join the Therapy Portal. Once you log into the Therapy Portal you will be able to start visits, make payments, and request future appointments.

How much time should I set aside for my appointment?

New Visit – 1 hour. Established Visit - 30 minutes.

The length of the appointment can vary based on your individualized treatment plan.

When will I get my prescriptions?

Prescriptions/refills will be sent electronically at the time of visit only. No prescriptions for ongoing Benzodiazepines will be ordered. This includes the following medications: Alprazolam (Xanax), Clonazepam (Klonopin), Lorazepam (Ativan) and others.

How can I get help if I’m in a crisis?

Can my provider complete disability forms?

The services offered by out providers are limited to medication management only. We are unable to complete Disability, Workman’s Comp, FMLA, Emotional Support Animal, Insurance, Academic/Employment Accommodation forms or letters.
